Hope all is well!
Last week I had the opportunity to meet with Ms. Guzman-Murdock, Principal of Wadsworth Elementary about coordinating the Big Smiles visit to Wadsworth Elementary scheduled for 2/12/2025 and Wadsworth EEC scheduled for 12/7/2024.
If possible, we would like to join the Wadsworth Elementary date on 2/12/2025 since they have more room and space to accommodate the needs required for the Big Smiles Dental Date.

1. Confirm the visit and let me know if field trips, activities, or early dismissals may hinder the visit.
2. Confirm if there will be a staff member to help.
3. Confirm you received your forms. Please communicate with parents that forms must be fully completed in order for us to verify them; name, DOB, and insurance ID # or SSN (if applicable), etc.
* Please pass out forms asap and fax completed forms to 248-919-4262. The bulk of your forms should be turned in no later than 2 weeks prior to your visit (if mailing, 4 weeks prior and copies should be made beforehand).
4. Your online link is also included; send an email blast with the link or post it on your school's website or other platforms (ClassDojo, Schoology, etc.) to increase participation.
